Hello,

I am filling out my AOS papers and I am slightly confused with all this I-693 and DS-3025 stuff.

I read that I don't need a new medical examination as my medical is less than 1 year old and that

I should send in a DS-3025 form.

I do not have this DS-3025 form! All I have is a copy of my entire medical in a sealed white

envelope that was send to me by the embassy in Frankfurt.

Is it necessary to include the DS-3025 form? If so how do I obtain it? I have my vaccination

booklet with all my vaccinations from Germany. All the vaccinations required by the government

were completed before my medical and my medical also states that I have all the vaccinations

that I needed.

It is probably in that extra sealed envelope that was provided to you. Not everyone receives an extra envelope with a copy of their entire medical inside. That is a rare thing. You can send that envelope with your AOS as is, or you can open it and send a copy of the DS-3025 only(it is part of the medical exam paperwork), and have a copy of your medical to keep. That is up to you.
