HELLO,

SENDING OUT MY AOS / IV PACKAGE ( TWO SEPARATE ENVELOPES IN ONE COMBINED PACKAGE) SOON. I HAVE NOT BEEN INVOICED YET FOR THE IV FEE, BUT HAVE IN THE PAID THE AOS FEE AND HAVE THE RECIPE WITH THE ADDRESS AND BARCODE THAT NEEDS TO BE INCLUDED IN THE PACKAGE. VIA MAIL I HAVE RECEIVED ANOTHER COVER SHEET WITH A CHECKLIST ON IT INCLUDED IN THE COVER LETTER.IT SAYS THAT IT ALSO NEEDS TO BE INCLUDED IN THE PACKAGE. SINCE I AM SENDING TWO PACKAGES SHOULD I COPY BOTH COVER SHEETS AND INCLUDE IN BOTH PACKAGES, IF YES THEN DO I CHECK THE MATERIAL INCLUDED IN THE AOS PACKAGE ALSO THAT IS IN THE IV PACKAGE AND VICE VERSA?

some have sent in the checklist sheet along with the barcoded cover page. some have not and have only included the barcoded cover page. there has been no survey completed to indicate which is 100% correct. if you have completed the AOS package and IV package properly AND included the barcoded cover page then you should expect to sail through the process. to include both pieces of papers will not hurt you or your application process.

I would check off the 'aos documents enclosed' and submit this in the aos package and repeat for the iv documents package. you've got both packages covered this way.....as you suggested. be sure to label both packages with the appropriate info prior to putting these in the larger mailing envelope. i was able to EP so I did not send in 2 checklists. I only emailed one.

you do not need to wait to pay the iv fee before mailing the packages. they are good to go once you have paid the aos fee. just thought I would mention this if you didn't already know.
