MEDICAL QUESTIONNAIRE London

Hi All,

I was looking at the medical questionnaire for London and I found 2 different versions

The first one I found here by selecting LND - London from the drop down menu

The Second one was from the London Embassy's page on the medical (http://london.usembassy.gov/immigrant-visas/medical-examination.html)

The questions are slightly different and the London Embassy one is dated 2012 whereas the other is 2014 but I thought I'd check to see if anyone knows which one to use.

I'd go with the newer one. They will hand you another form to fill out at the clinic and the doctor will quiz you on some of these topics so I don't think choosing between these forms is deal breaker whichever one you take in.

Regardless of which questionaire you use, there will be another with more specific questions to fill in once you arrive at the doctor.
