Report of Marriage Form ( Question )

Hi Everyone,

I am about to fill out the ROM form. Question:

1. For the info's on Wife - I assume it's the maiden name (?) that is being asked.

2. Age - is it the age when you got married or the current age as you file the form?

3. Citizenship - for my USC spouse - do I have to place USA ?

4. Residence - is it the current residence or the residence when you got married? ( I am asking coz we are now living in a different state from where we got married ). Do I have to place the full address or just the state is ok ( i.e Florida, USA)

5 Name of witness: Do we have to provide 4 different witness and get their address? 4 because there's like witness 1 & 2 for husband and another witness 1& 2 for wife.

6. Civil status- is this the civil status before getting married?

Sorry to ask - I just want to be sure that I am providing the correct info. I can't see any instructions for this form.

PPS.....we have been married since 2011 and I am just reporting it - do we have to send a letter ( notraized ) explaining why we just reported our marriage now? To those who reported late - what was a good reason - I was in hiatus on this just because I was lolz :D

Maiden name for your wife. Current age. Civil status was before you were married. Current residence, city-state (?) Report of Marriage between Filipinos or a Filipino and a Foreign National

The marriage of a Filipino should be reported to and registered with the Philippine National Statistics Office through the Embassy/Consulate General having jurisdiction over the locality where the event took place. Please check the Consulate Finder or the Consulates Link for the states covered by the Embassy/Consulates General in the U.S.

BASIC REQUIREMENTS:

  1. Four (4) original duly-accomplished Report of Marriage Contracted Abroad form, typed or printed legibly in black or blue ink, signed by both husband and wife. Wives should indicate their maiden name as the last name. If submitting by mail, the completed forms must be notarized.
  2. One (1) original or certified true copy and four (4) photocopies of the Marriage Certificate or Contract (original will be returned)
  3. Four (4) photocopies of a government-issued identification card or passport of both husband and wife
  4. Four (4) photocopies of Philippine birth certificate of Filipino spouse
  5. Self-addressed return envelope, with appropriate stamps for express or priority mail with tracking numbers via US Postal Service, or with pre-paid mailing envelope from private courier of choice, if Report of Marriage is to be mailed back.
  6. Processing fee of $25.00 (non-refundable), payable in cash, or money order, bank draft, certified check or cashier’s check, made payable to "Embassy of the Philippines" (or "Philippine Consulate General", if application is made at one of the Philippine Consulates General in the U.S.). Personal checks and credit cards are not accepted.
  7. For applicants residing in U.S. territories or other countries within the jurisdiction of the Philippine Embassy, applicants should enclose a treasurer’s, manager’s or certified check issued by a local bank that has a corresponding bank in the U.S., payable to the courier of choice, in U.S. dollars, to cover cost of mailing, and a corresponding self-addressed courier’s address label. Personal checks are not accepted.

When we got married it was only me , my husband, and the judge...so not sure what to write on the witness part.

Hello, can I ask question too? I was confused because the latest website of philippine consulate in Washington, D.C. Doesn't require 4 passport size photo for both of us but the others says that it is a requirement? Which should I based my requirements? And why I can't open the for for report of marriage it says URL not found. Thank you.

That should be fine, just leave the rest blank, don't need to put NA

1. what to put id # and foreign service post?that is located on top of rom form

2.about witnesses.it was only me and my husband wgen we got married and we got random people for our witness and on our marriage license it was only signature no full name and addresses.can i put n/a?it says on the form dont leave any blank space.thanks pkease kindly help

  1. That area of the form, where the ID and Foreign Service Post is, is for the Embassy to fill in. You can leave those blank.
  2. Just add N/A to those blanks where they are asking for the witness' names. I would attach a note that only the signatures appear on your marriage certificate and that there is no printed name available.
