When I look up the travel history of my wife on the I-94 Information page, I can see when she first arrived in the USA, in March 2013, I see an entry when we left in March 2014 to go back to her country. She already had her Conditional Green Card when we left in March 2014. What I do not see is anything about her return to the USA in April 2014. Is this going to be a problem? We are planing on another trip to Iran in May 2015 and I want to find out if she will have any issues when we return. We went though Immigration together in Chicago, first stamped my passport, then looked at hers, took her picture, compared her picture to the one taken when she entered in March 2013. I joked with the agent and asked if she was the same person. He then stamped her passport and we were on our way. But now I do not see a record of that entry for her. What can we do?

I have no answers, however border agents are not known for their sense of humor. I wouldn't joke about false identities, especially for an Iranian.

You should likely contact a CBP Deferred Inspection Office to inquire about correcting the !-94 record: http://www.cbp.gov/contact/deferred-inspection/overview-deferred-inspection
