New DS-3025 Vaccination Documentation Worksheet?

I will get my vaccination records sealed by a panel physician. I was Googling the form and found one that is the same but with different design. I also says the expiration date for the one I found is 2017. Is that the same? Does this mean that mine, whose expiration date is Aug. 2014 isn't valid anymore? Just curious. Timeline

If the DS-3025 used at your K-1 exam was current at the time, then it will be accepted. Also, the expiration dates on the forms are usually just for the USCIS or DoS to know when an update may be needed, and a lot of the time past form editions are still acceptable.
