Help filing for I-693 k1 process to AOS

Hey everyone so my previous post was asking what to do since my husband had all his vaccinations under DS3025 but we couldnt find paperwork to prove ..Well now we found the copy of DS3025 with all vaccinations he had taken including a CD with ressults of his radiographs as well that has a sticker that say do not open....Now that we have prove and was vaccinated less than a year ago,what should I do?,do I send just the copy or will I have to fill something else to include form Ds3025 :dancing::idea::clock: Please help us this is the only step left to do. :reading:
