I am reporting my marriage at the Philippine Consulate in Chicago and need help.

1.How do you notarize the Report of Marriage Form? Where will the notary sign in the form?

2. Do they need photocopies of my husband's birth certificate? I am confused as it is not specified if they only need my birth certificate in the requirements on their website.

Notarize anywhere on the RMF. We used the right margin and had it notarized by our bank as they do not charge for that service.

It does no say anything about your husbands birth certificate but I would bring copies and an original just in case. Originals are usually returned.

Check Cashed Dec 21, 2015 Check was cashed

NOA-1 Issued Dec 21, 2015 NOA-1 for ROC issued

NOA-1 Issued Dec 26, 2015 NOA-1 Received

Biometrics Appt. Jan 29, 2016 Biometrics Appointment Scheduled [Completed]

Timeline

I gave them a certified copy of my birth certificate from the New York CIty Department of Health Services and a certified copy of the marriage license from the town that issued it. They were accepted fine, no notary required.

The actual Report of Marriage forms are required to be notarized unless you sign the forms at an outreach event or at the consulate

I'm still trying to find a notary that will do it. The ones at the banks I've been to won't notarize the forms because I'm not an account holder, I kinda understand that one, or having no specified place for it on the form according to their legal department :huh: Just a matter of time before I do find a local notary, outside of the banks, that will.
