Derivatives are people who are qualified for a visa because of their relationship to the petitioner or the principal applicant. They can be the spouse, children, parents, or unmarried siblings.

So example for K-1 visa applicants, their derivatives are their kids who will apply for a K-2 visa.

The companion must also present a government-issued ID as proof of identity.

for number 7, the one with the red, do they mean the ds-160 form?

my k1 visa interview will be on the 10th of December. would it be okay if I have the medical on Monday 17th of november?

Embassy states to do the medical at least 7 days prior to the interview, so you should be just fine.

Keep in mind that the validity of the visa begins from the medical exam and not the interview. You will have 6 months from your medical date to depart to the USofA.
