I have copy of DS-3025 with all vaccinations done. Do I still need "I-693 transcription" thing done?

Hi All - I am putting together my AoS application and am VERY confused about the whole medical report thing. I am from UK, and I have a copy of my DS-3025 in which the doctor entered all the vaccination details with dates, and ticked on the box where it says I have completed all the vaccination requirements. I am going to send a copy of this with my application. But I see so many people getting some sort of "transcription" done for the I-693. What is this? And do I need it too, even though I have completed all vaccination requirements before I got here (two months ago), and have a DS-3025 which says as much? PLEASE HELP! Thank you

11/07/2014: K1 visa received

11/08/2014: PoE

11/13/2014: Marriage

11/14/2014: AoS/EAD/AP application mailed

(11/19/2014: Notice date on I-797 NoA)

11/20/2014: Email and text acknowledgements received

11/24/2014: NoA hardcopies received

11/29/2014: Biometrics appointment letter received

12/02/2014: Biometrics completed

01/27/2014: EAD & AP approved. Card ordered

02/02/2014: EAD/AP combo card mailed to me

02/04/2014: EAD/AP combo card received by USPS post

02/18/2015: Notice of Potential Interview Waiver

07/27/2015: Welcome notice was mailed

you only need a 1-693 when your vaccinations are incomplete. Since you already have all required vaccinations you only need the 3025 to send for AOS. Make sure you make copies of your 3025 for your own files. Hope I helped.

K1s don't have to complete their vaccinations in order to get a visa. Those that do it later in the US, or even after their medical by their NHS doctor, must get signed off on a vaccination form (I-693) by a civil surgeon for their adjustment of status If you had all the required shots before your visa medical, then you got the official sign-off by Knightsbridge clinic (Panel physicians).
