I am not sure if i ned to fill this out. I do not even know how to fill it in Chinese.

I have sent packages to my fiance written in english and she has received them fine.

Do i need to do this?

Were you Chinese born and had Chinese name? If not, you can ignore it for your part,

but if your fiancee is Chinese and have Chinese name, she should be able to fill-in or supply you Chinese characters.

You can just copy & paste it in the doc.

In order to avoid any delays, you need to follow all the instructions,

and I would not skip section.

[2014-10-22] Notification of SSC in mail, will arrive "within 2 weeks"
