confused about vaccination help

i got my ds 3025 record of vaccination but then i dont have MMR shot because i was pregnant that time. so now i need to get a shot here in the US and flu vaccine also. Now my question is, can i just go to any physician that are NO LESS THAN 4 YEARS OF PROFESSIONAL Experience, to have a shot and let them fill up my DS 3025?

What does the blanket designation of health departments as Civil Surgeons mean?

The blanket designation of health departments as Civil Surgeons applies only to the vaccination assessment and only to refugees. Also, only health departments that have a physician or physicians meeting the legal definition of Civil Surgeon can participate in this designation, and accepting the designation is entirely voluntary on the part of health departments. A Civil Surgeon is legally defined as a licensed physician with no less than 4 years of professional experience.

The completed I-693 medical examination form must contain the official stamp or seal of office and be given to the refugee in a sealed envelope for presentation to the U.S. Citizenship and Immigration Services (USCIS). The form must also be signed by a health department physician who has at least 4 years of professional experience.

or i need to have I 693 to them to fill up ? do i just need the page 1 and page 5 of that form?

Only a designated civil surgeon can fill out an provide an I-693 for you. You are not required to get another medical if yours was done within a year of filing for AOS, but you will need the missing MMR and to have the vaccinations transcribed onto an I-693. You will need page 1 and the vaccinations record page, which I believe is page 5.

You can also get the shot elsewhere if it is cheaper and take that record and your DS-3025 with you to the civil surgeon.

thank you Kay Dee Cee for the responce. oh okey so i would just gonna get a shot in health department because it might b cheaper but then i still need to see a civil surgeon to transcribed a i-693 for me.

Yes, you will need a Civil Surgeon to transcribe your vaccination record to the I-693 if you get your shots from elsewhere. I suggest you call a Civil Surgeon's office to inquire how much they would charge for transcribing.
