On my husband's DS-2053 form under Varicella it says Not Routinely Available. He has not had chicken pox, so does he have to see a civil surgeon now????? Also, it is Influenza season so does he need to also see a civil surgeon for this???? OR can he just get BOTH shots anywhere and they can mark it on his DS-3025 form??? Thank you!

The DS-3025 is provided at a K-1 medical exam. No one can mark anything else on that form. It is done. He will need to see a civil surgeon to get the vaccinations transcribed onto an I-693. He will need the flu shot now, since it is flu season and he needs to get an I-693. You can get the shots elsewhere if they are cheaper than at the civil surgeon. You would just bring your DS-3025 and the records of the 2 new shots so the civil surgeon could mark them all off on your I-693. http://www.visajourney.com/forums/topic/428381-i-693-report-of-medical-examination-and-vaccination-report/?p=6206232

Two more things, sorry. Do you have any idea how much it will cost to see a Civil Surgeon to do this? Also, how can I find a Civil Surgeon close to our area?

They can charge whatever they like, so you will need to call around. Some will insist on giving you a medical you do not need. You need to let them know you are adjusting from a K-1 visa and do not need a new medical exam, just the vaccinations transcribed onto the I-693. There is a very large thread here on VJ. You might be able to find a civil surgeon near you by looking through it > http://www.visajourney.com/forums/topic/231026-civil-surgeons-who-dont-rip-you-off/
