Hi all,

I am getting ready to file for my wifes AOS. So I need your help:

1) She is from the Ukraine and in the Russian world your second name is your fathers name.... In her passport this name is not represented. Since her passport is her primary identification now, what names do I use on all the AOS formS... Her married name with or without her so calles second name?

2) G-325A is 5 identical pages... Do I fill out ALL OF THEM?

3) I-639 (the medical)... aslo multiple identical documents. I do not fill out anything on this, right? Have to take it to the doctor? Correct?

I have the following documents completed: G-325A (Biog)... I-485 (PR AOS)... I-864 (Affadavid OS)... I-693 (I think I do not fill this one in)... What else do I need??? I know I need some coer letter... my tax returns... what else????

Use any names shown on her birth certificate (minus her surname if she is taking yours as her new surname)

2) G-325A is 5 identical pages... Do I fill out ALL OF THEM?

You need to submit all the copies, I had one with carbon paper and another I filled out online and then photcopied 3 times and signed them seperately).

3) I-639 (the medical)... aslo multiple identical documents. I do not fill out anything on this, right? Have to take it to the doctor? Correct?

Yes, take it to a civil surgeon. They will fill it out.

You don't need to use a cover letter unless you want to. It can help to keep your application organised and for you to check to make sure everything is there and in order.

Go check out that ^^^^ link. It lists the things you need. Also read the instructions on the various forms, they list what items need to be sent in with the application.

G-325A is 5 identical pages... Do I fill out ALL OF THEM?

If you fill it in online you fill in once but when you hit print all 4 pages will print. ( they will all be filled in)

All 4 pages must have original signatures.

Hi,

I would suggest using a cover letter as it is a correspondence standard. This is what I have posted before -

We set up the cover letter so they could easily see the list of applications and support documents, etc. Everything inside was in order to the list. The applications (AOS, AP, EAD) were seperated from each other with post-it tabs with the application type written at the top to be in view. My husband even post-it tabbed his passport pages with entries into the US! Here is the letter:

Some other notes:

My husband already had an SS# as he was here previously on a J1 work visa.

Photos were put into ziplock bags with white labels on them with his name, A# and SS# (this info was also written on the back of the photos). I used the white labels cuz writing doesn't really show up on the clear plastic.

We almost sent in the DS-3025 instead of the I-693A due to difficulty finding a reasonably priced CV, but then thought better of it and got the I-693A done. I got the feeling sending the DS-3025 would scream RFE, even though some people have gotten through with only the 3025.

The copies of employer and bank letters were from our K-1, supported with current documents. I couldn't deal with getting updated letters for the I-864.

I really think the best we did for it was the organization. I think it makes their job easier and in some way they appreciate it. It might also determine if you get transferred to CSC. I get the sense the "good" apps are being sent there, hence the quick, non-interview approvals that people are getting.
