My fiance will be filing for adjustment of status from a K-1 next month, and we have a lot of confusion regarding what should be done with form I-693. He just had a medical done in August in Ireland for the visa and has a completed DS-3025 as well as a complete vaccination history from the department of health in Ireland. I have read that we need to get a Civil Surgeon to transcribe only parts of the I-693 and sign it because he's already had the medical. My question is what exactly should we be asking the civil surgeon for and what price should we expect to pay for those services (I know it varies, but I'm just looking for a reasonable estimate)?

Here is a very blurry copy of his DS-3025 that states his vaccination history is complete and satisfactory at the bottom.
