Last Name(at Parent's birth ) - For mother's information, I assume last name as maiden last name.

Does this mean First/middle name also maiden name?

Yes, put in your mothers maiden name.

I am not sure what you mean by "Does this mean First/middle name also maiden name?"

First and middle name does not change when someone gets married unless in some cultures it does.

I would not pay all that much attention to this. Make sure your name is 100% correct. They probably wont run a background check on your mom and dad.

Write your mother's name as it is on her birth certificate... if any of her names, first, middle, or last, have changed since birth, write her original name at birth.
