i have e-passport btw..so what does --->photocopies of a government-issued identification card or passport of both husband and wife

is this different of what we've got..or there is something special that need to do to our passport for that??

4 copies of my birth certificate.. correct??

which one im missing..and which is not right?

and btw..

and lastly...

the treasurer's manager.. or certified check issue by the local bank..

how and where to obtain this..if personal check is not accepted..?

Go by what is posted on the consulate website that serves your state. Believe it or not the requirements are not identical from each consulate.
