Vaccination records question

My wife is beginning to gather required documents for an eventual embassy interview. Her original personal vaccination records were washed away during Typhoon Pedring in Oct 2011.

When she recently went to the health center in Navotas to get a replacement they asked her age (24). Then told her they only retain records for 20 years!

What other options does she have to get a replacement document?

I and she would of course prefer not to have to go through the vaccination process again but if she does, what are the minimum acceptable immunizations required?

Don't worry about it, not needed at all. You are fine.

She has to have the shot at SLEC. Usually it is only 4 vaccinations. Info in the embassy instructions letter. Follow my link below

The medical staff at SLEC will work with the applicant to determine what they have received and not received through a series of questions. The panel physician will determine which of the vaccinations are medically appropriate for you, given your age, medical history and current medical condition.

What do I do if the applicant does not have a vaccination record?

The panel physician will work with the applicant to determine which vaccinations the applicant may need to meet vaccination requirements.

I didn't have any with me. They gave me 2 shots (MMR and TDap). It's supposed to be 3 but they said that there was a nationwide shortage on the rubella vaccine so they waived that.
