I 693 form

My wife came on a K1 visa,I read wonly need to fill out part 1 of the I 693 form,however that part says to go to a surgeon to get the vaccines put into the I 693 form,do I really need to do that or just attach a copy of the vaccines records to the I 693?

In summary, she doesn't need to see a CS if her overseas medical was taken within the year prior to the I-485 filing and all the required vaccinations are given and well-documented on her DS-3025.

I am not sure,I remember seeing X's on different columns,not sure if every vaccine listed on that form is required

Have a read on the link in my first post. The required vaccines are listed there along with other details that will help you decide whether or not there is a need for your wife to see a CS.

We never received the DS-3025 at POE and only have a yellow card with several vaccinations written on it and a chest X-ray. So we have no idea whether or not all the vaccinations were done properly. From what I gathered from researching this topic on VJ is that I can skip the I-693 since I'm a K1 and hope that the CBP or USCIS didn't lose the DS-3025. If the DS-3025 was lost or they determine it is incomplete, I will be sent an RFE in which case I will need to bring the I-693 to a civil surgeon and complete an entirely new physical, prices will vary depending on civil surgeon.

So I'm assembling our AOS and preparing to file next week without any medical information included. If I'm making a mistake hopefully someone will let me know before next week.

if the DS 3025 is lost then you will probably have to have the vaccinations transcribed on to a 693, you should not need to have another medical if the one you had is in date.

A copy of the 3025 is normally handed over at the interview not POE, the original 3025 should have been in the envelope handed over to CBP at POE, they dont give you anything
