Yes i did but the doctor that was stated is not doing immigration stuff anymore. So sad

I have also called almost all the listed CS in NJ when I was preparing for my AOS, so I hear you. As advised by Kitthekat, contact your local health department for the Tdap vaccine, as they normally give way way cheaper prices for vaccines than the CS. You can simply bring this vaccine record plus your DS-3025 when you go to the CS for the vaccine transcription.

I had my wife redo the entire physical and after we consulted with the civil surgeon

it was the right thing to do. There are shots she needed and tests that needed to be

done even though she had one in in the Phils only four months prior. It cost us

600 dollars and they said my insurance wouldnt cover any of it. But, because the

hospital had signed a contract with Blue Cross Blue Shield as a preferred provider

they had to settle for less and yes they did cover the physical. Good Luck....

No, they are really picky on that.

My clinic wasn't even on the USCIS site, and they do the immigration papers constantly. Many of those who were on the site, were no longer active. Shop around, I was quoted absurd prices as well, and ended up paying 80 bucks (60 for the papers, and 20 for the two vaccines I needed). Mine actually had a separate department that dealt with immigration and all the medical issues related to it.

02/25/2011 Married!

03/19/2012 FINALLY submitted the AOS paperwork: I-130, I-485, I-765, etc.

03/23/2012 Received notification that USCIS has received everything.

03/26/2012 Biometrics Appointment Notice: April 23, 2012.

04/02/2012 NOA's received. Now we wait...

04/23/2012 Biometrics completed.

05/18/2012 EAD card mailed but not yet received.

06/06/2012 Rough interview but we think we're approved! Website says i485 status "Card Production".

06/12/2012 We received the "Welcome the United States" letters! Website says Green Card was mailed.

LIMBO: Green card lost in mail. Facing the possibility of paying hundreds in fees and basically repeating the entire process.

All good now! USPS lost it, then found it.

04/24/14 Back to VJ to get help with I-751, Removal of Conditions.

05/31/14 Mailed I-751 finally, with less than a week to deadline!

06/02/14 NOA1 (that was fast!)

07/01/14 Biometrics

7/25/14 Approved for Removal of Conditions

Thank you so much for your reply guys. I think i need dtap, i ended up having an appointment to the cheapest i can get. 125$ for everything. And my vaccination will just be in the county because thats the most decent price I can get. I

One in Oak Park, MI charge $100 cash up front to do the form and basic physical exam and extra $20 (around that, bill later)for blood drawn.

No, since you were not given the Tdap vaccine, you will need to get that shot and get the vaccination records transcribed to the I-693. Check with your local county health clinic to see if you can get the shot cheaper than at the civil surgeon's.
