Hi,

Does anyone know of a civil surgeon doctor in New Jersey or New York City area that knows about the K-1 AOS process? So far all of the doctors that we called are a bunch of crooks that what to preform a physical and blood work. The I-693 instructions clearly states that a physical or blood work its not need if the K-1 visa holder had one done less than a year of filing the AOS. The physical and blood work were don in Feb 2014.

All we need is the translation from the State Departmenmt form DS-3025 to the USCIS form I-693. The vaccination record from the state deparment for my wife and my step-son states that all shots are completed series.

If all shots are completed, you don't need the form I-693. Just send a copy of vaccination sheet.
