Do I need I-693?

I read the previous thread and wanted to ask my own question regarding my DS3025. Would be rude for me to post my question on someone else's thread.

My DS-3025 stated Complete, all requirements met

Td, tdap, not routinely available

Varicella, not routinely available(had chickenpox but doctor didn't ask)

MMR, vaccine given by panel physician

All the others are marked not age appropriate.

Medical done in December 2013 so it's not a year yet.

Do I need to transcribe or will I be ok?

I have heard different answers on that, most people say to file a copy of your DS-3025 and that is fine. I didn't file anything because I was never given a copy, everything was given at POE. So if you have that, just copy it and put it in with your paperwork

It is not marked off that you had Tdap or TD, nor is it marked off with a VH for varicella history or that you had the vaccine. You did not have all the required vaccinations marked off on a properly filled out DS-3025, therefore you need to see a civil surgeon for the missing vaccinations and transcription to an I-693. http://www.visajourney.com/forums/topic/428381-i-693-report-of-medical-examination-and-vaccination-report/?p=6206232
