Does the embassy provide form for the parental advice or you have to make your own letter?

No parent advice needed ath the US Embassy. Just need to be 18 and over. If 18, the US Embassy may ask to tlak with your parents, but doubtful. Parent advice or parent permission is ONLY required by the Philipines and CFO.

Most local registries/city hall have forms for ADVISE and/or CONSENT

Print and fill out. The form needs to be notarized and bring a copy of the parents ID.
