G325A Using a seperate sheet of paper

While filling out this form, if i need more room, they say to use a seperate sheet of paper. There is no item number for some boxes, and what would i put in the box to refer to the sheet? See Attached sheet?

on the form type see attached. On the attached type out the title of the field.

5 years of employment or whatever the title is in bold. and type away. I used one no issues.

I filled most of the boxes out on a separate sheet of paper as well, just make sure that the paper is labeled appropriately and that you do number/title your answers.
