I decided to take my husband last name, but we do not know how to fill the forms either with my original name, what they have on file our with my new name that I would have to change first.

Also Im from Mexico so my passport has my mexican name and I obviously do not want to have any kind of problem with a green card with a name and a passport with an another one.

If you want your green card to be in your married name, then fill up the forms in your married name.

You could always change your passport to your married name later on.

My passport is still in my maiden name, I have no issue entering US. I just have to present my passport, green card and marriage certificate to show my name change.

Done with K1, AOS and ROC
