Our AOS interview is Wednesday and I see that an I-693 form and vaccination suppkiment is listed if it had not already been submitted. I assumed that this was in the sealed packet that my fiance handed in at the airport but after reading the forums I am not 100% sure this is the case. Whats the story on this form, do I need to do something or not?

I-693 is not the same as the DS-3025, which is your fiance's vaccination documentation worksheet.

Where is the I-693 listed, by the way? If listed in the forms needed to bring for your interview, you might need to see a civil surgeon in your area: https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV

Its listed on the I-797 for the interview but it says if not already submitted which I assume we already have done so I think its boilerplate, not something specifically aimed at us. Ive talked to two others who went through the same office without needing this form so I am hoping that it will be the same for us.

My letter also says this. I sent a photocopy of my complete DS-3025 when I sent the AOS paperwork, so now I'm unsure if I need to get an I-693 done or not.
