This is a first for what I see them requesting for...

Petitioner and beneficiary : provide information about ex spouse/ ex boyfriend full name, birthday, place of birth and current address and landline phone number. Provide evidence of address. Must be notarized.

I don't speak with my baby daddy often to where I can get that information. Anyone knows how to handle that if I can't get proof of address?

Beneficiary: medicinal exam results not received (he went about a week/two weeks prior)

I think if you dont know where he lives now, then you can write a letter to explain the reasons and notarize it.

Credit card can be obtained from your online banking. If you dont have it by any chance, then you can go to your bank to request ( they will charge money ) .

The result of medical examination can be obtained after 2-3 days. He should contact Cho Ray or IMO.

My baby daddy is currently in Houston tx, but he's supposed to be in New Zealand. I don't have his address in NZ but I have his address in Houston. But I'm not sure if I can get proof of it. We don't talk much :/

Do you know his phone number in Texas?

Any his friends know his phone number?

if you can contact him, then it would be easy to ask for the proof.

Ask him to scan/ fax his lease or his mortage, electric,and water bill.

I have it but that's just it. The utilities are not in his name. He stays with his mom. He moved to NZ a few years ago. He came back to the states for his sisters funeral and now he is not able to get back into NZ because his visa expired for them. He's stuck here.

Can his mom write a letter that says he lives with her and sign it? Then, she can give her lease, electric bill, so on..

You still have plenty time to submit those additional documents because Embassy will not work after tomorrow until Feb 6

I have his license which shows his address here locally in LA...but i will explain in detail in the letter why I do not have a current address for him and the circumstance as to why that is.

I've encountered another problem with the credit card statement.

They need to see exactly who the card is issued to and date of purchase and price of purchase.

The card used was for my dad...BUT the portion that shows that information was sent in with the payment back in September 2012.

Now I'm wondering if I can send a copy of the statement showing the account number from the date of purchase AND a recent copy of a statement from the same credit card company showing its in his name. I tried explaining it to the fiance but i just got aggravated with explaining it to him. He doesnt understand argh!

You can explain that you only know his old address. He moved to NZ and you are no longer knowing his new address in NZ.

It is not really matter who that credit card is? You dont have that portion because it goes with the payment. However, you can ask your dad to go in his online banking to obtain it. My statement is still online from 2008- Present. By the way, I have both Chase and Bank of America account and it does work for both.

If somehow that you cant obtain by going online, then go to local branch bank to ask for it. They will charge money and it is somewhere around 10 dollar/ statement.

Can you please elaborate on this statement? The airline ticket for whom? The beneficiary? If it is for the beneficiary, do they expect you to purchase the ticket for your beneficiary when you don't exactly know when he will receive his Visa?

She did write down Petitioner: ..... They asked the petitioner to show the proof that she bought airline ticket to VietNam for visiting her fiance ( not beneficiary ).

I did contact the cc company. Online banking is only available for a year. They want to charge me up to $30/hr of research for locating the statement and $5 for however many months I need

They tried to make money out of you and I dont believe it will take them more than 30 minutes.

However, it is hard to know how many hours they put down. If you still have the bottom portion of the credit card where it shows the amount that was purchased for airplane, then I would put the credit card on top near the bottom portion of statement and scan all in one.

Are they always asking for this information for an exbf/gf, or only when a child is involved?

I think each case is different from one another. However, It is better if we have everything ready.
