Hello! I am trying to get my head around the medical requirements for AOS. The checklist says this:

"I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. Regardless of if you are exempt from a medical exam, you are still required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you."

I had my medical exam in London in August. So that means I don't need another exam, right? But I still need a civil surgeon to fill out the rest of the form?

I do not have DS-3025 like the above suggests, but I have the form the doctor gave me at my medical in London, as well as a printout of the immunisation record from my old GP in England.

Should I then include those documents (the vaccination record and the one from the visa doctor) in my packet for AOS?

If you do not have a DS-3025, you must see a civil surgeon and have your vaccinations transcribed onto an I-693 form. You will include the I-693 form with your AOS packet.

The form that your doctor gave you at medical is probably ds-3025, does it have all vaccination information on it etc (should say ds-3025 in bottom left corner from what i remember)?

If your vaccinations are complete, i.e there are either ticks in "age not appropriate", "vh" or date of vaccines then you do not need anything else, just take a photocopy of that form and send it with your AOS paperwork.

If you were told at the medical that you need more vaccines then you'll need civil surgeon to transcribe you vaccination history for AOS.

Ah yes, the thing I have has DS-3025 in the corner! So I just photocopy this sheet and mail it with my AOS? I don't have to see a civil surgeon? Or I still need to get a civil surge to sign it?

If your DS-3025 is complete, you just need to send a copy of this with your AOS.

A completed DS-3025 will have the following:

  • All vaccines in the top section marked as either: completed, not age appropriate, or insufficient time interval
  • At the bottom it is marked as either COMPLETE, or INCOMPLETE but may be eligible for a blanket waiver
  • Signed by the Doctor

Yay one step closer to saving some $$$ and civil surgeon hassle. As per Jak-kay recommendation, check that post and you'll know for sure if you can just photocopy that page.

I went to a designated civil surgeon today and he ask me to get shots for Hepatitis B shot and Varicella even if I had Varicella history, do I have to explain that I don't need the Varicella shot anymore? What if he insist to get those two shot? Do I really need Hepa B and Varicella shot? He mention that the immigration doesn't accept the "not age appropriate" reason anymore that's why he told us to get those shots. Is it true? Your answer is highly appreciated.

And no, that is not true. He is trying to get money out of you. Perhaps find a new civil surgeon.
