The online application has a spot for Surenames and a spot for Given Names. Do I include the middle name for either of these? My fiancee's passport lists her middle name clearly seperate from her Surname and also from her Middle name.

Do you mean Surname?Surname is (Family name) Given name is ( First Name).. Just Write on appropriate Box.

Philippine Name Has a format of - First Name + Middle Name + Surname

The reason why we have middle name because that is the Surname of our mother Before Marriage (Maiden Name)

ex. Juan dela Cruz

Juan -is the ( First name/Given Name )

Dela-is (Middle Name)

Cruz- is the Surname (Family Name)

On her Passport, it states under issuing authority DFA TACLOBAN.

Though all passports come from Manila.

And she picked it up in Cebu, due to being displaced with Yolanda.

So, Should I put Tacloban since that is the Issuing Authority on the passport?

I don't mean to keep making seprate threads on the DS-160. It's just that I can't see the next step until I fully complete the Current step on this online form!

Regarding Intended Date of Arrival & Intended Length of Stay in U.S.

Should I say that she'll be here for 3 months (the lenght of the Fiancee Visa), or 10 years (as she will be living in the USA after marriage)?

Thank you in advance!

Keep in mind that you are applying for Non-immigrant visa K1. you should not put there 10 years. coz it will referring to an immigrant visa. so just put there 3 months.

I'm a bit confused. Even though the K1 visa is technically a non-immigrant visa, it seems as though this process is treating it like an immigrant visa. In fact, the US Embassy in Manila's website talks about Fiancee Visas on their Immigrant Visa page, but not on their non-immigrant Visa page. http://manila.usembassy.gov/wwwh3023.html

So, how long should I put? 3 months? 10 years?

Ya know it really doesn't matter. The DS-160 is not designed with the K-1 visa in mind, it more of a make do form with questions that don't necessarily pertain to the K-1.

i'm just a little confused... i'm done with my DS-160 and i couldn't remember answering a question pertaining to the "intended length of stay in the US" and as what i do remember, its just the "date of arrival". please correct me if i'm wrong.
