I have two questions on the I -134, First on the I 134 form question 10 I have submitted a visa petition to USCIS on behave of the following person. I am thinking that I have to put my fiancé name there is that correct ? and for the date submitted do they want the day I sent the I-129 F or the day they received it . Also a question as to where to send the I 134 when completed I read the instructions and could not find this , do I send to my fiancé or to the consulate. If any one can help me I would be very appreciative. Thank you in advance river 56.

question 10 - yes put your fiance's name on it. refer to the sample form: http://www.visajourney.com/examples/Form-I-134.pdf

and for the date submitted do they want the day I sent the I-129 F or the day they received it - You can use the date from your NOA1 receipt.

Also a question as to where to send the I 134 when completed I read the instructions and could not find this , do I send to my fiancé or to the consulate. - You bring the originally signed form to the interview, including the evidences such as W2, ITR, letter from employer, pay stubs etc.

First: Your fiancee will need your ORIGINAL, SIGNED I-134 for her interview, so send it to her.

Line 10...yes, your fiancee's name, followed by in parenthesis "(subject of this affidavit)".

Date submitted??? I put date sent to USCIS.

Question 10 is your fiance's name, the date submitted is your NOA1 date.
