I am completing the I 134 now that the consulate has received the forwarded petition. I have looked over the guide and sample for the I 134, but have many questions I want to get answered before it is submitted:

1. Next to the section at the bottom where it says "I acknowledge I have read "Sponsor and Alien Liability" on page 2" there is a space next to it. Do I put my signature here, initials, or leave it blank? The space is small and I am uncertain. Right under this section is a space for my signature that says "signature of sponsor" that is clearly marked.

2. I am listing my income and money in my bank accounts. Do I need to get letter from my bank to submit with this form or is the monthly mailed bank statement enough? Also, do I need a salary letter from my employer? I have tax transcripts from the last 3 years.

3. Does any of this, including the form itself, need to be notorized?

4. Question #11: I read somewhere that I should NOT check "intend" or "do not attend" and just write under the space provided "N/A K1 petition for permanent residence." Is this true? It seems strange/incomplete to not check anything.

July 30, 2014: Entered United States on K1 Visa

August 15, 2014: MARRIED

October 16: NOA 1 for I485 and I765
November 6: Biometrics
December 30: EAD approved and mailed

1) I put my full name.

2) If you are above poverty guideline then you dont need the letter from your bank. You should get the letter from your employer.

3) No.

4) I followed the example forms in VJ.

You cant fill I-134 right now because the old form will expire on 12/31/2013 and of course you will not interview before that date. Just wait for the new version of I-134.

New Journey AOS:

My fiancee came to US on the 8th of March, 2014 under K-1 visa at Newark, NJ Airport for POE.

Applied Social Security Number on the 10 of March, 2014.

Married on the 13th of March, 2014.

Sent I-485, I-765, and I-131 on the 15th of March, 2014.

Received EAD on June 7, 2014.

Received Notice of Potential Interview Waiver on July 1, 2014 date 6/27

Contacted Ombudsman on 01/12/2015.

Ombudsman contacted USCIS on 02/20/2015.

Ombudsman sent a follow up to USCIS on 03/18/2015.

I-485 approved on 03/31/2015 ( we ordered your card).

Welcome notice was mailed on 04/01/2015.

Received Welcome Notice on 04/08/2015.

Card was mailed to me/picked up by USPS on 04/08/2015.

Received GC on 4/10/2015.

You cant fill I-134 right now because the old form will expire on 12/31/2013 and of course you will not interview before that date. Just wait for the new version of I-134.

I didn't realize that there is new I 134, so I will have to wait, I guess.

I'll ask my employer for a letter this week. I have my tax transcripts, too, but I guess this won't be necessary since I am well above the poverty line.
