I have to change my name on my Passport. I married my husband in February 2012.

I am a USC Female. my US passport has my Birth name "Mital Naren Patel".

Now I want to change my name to "Mital Hardik Patel". In our Culture we change our middle name after marriage.

I contacted the Passport agency and Asked them for the process. they sent me this:

"The name on the new passport must be exactly the same as on the previous
passport, unless accompanied by a name change document (court order or
marriage certificate only). If you are unable to document the name
change in this manner, you must apply in person using Forms DS-11
Application for a U.S. Passport. You will be required to submit documentation that you have used that
name exclusively for 5 years."

I do have my marriage certi. But, since we do not change the last name can someone tell me what the process is for the middle name change? what documents are needed to support the name change?

Any help would be appreciated.

can you make your maiden name your middle name? I think they can only change it to what ever on your marriage certificate and since you dont have proof to show that you have use your married name it will be difficult. Thats just my opinion not a fact. You need to tell them its a cultural thing and you need to show that to them.

Is your social security card in your new name? and your drivers license? If yes then send the certified copy of the marriage certificate/license as well as copies of these documents (that show the correct name) in with your passport for updation.

If those other documents aren't updated you need to start there first (SS card then DL) and then you should not have a problem updating the passport.

Marriage is one of those rare occasion when you can change your name without any major legal paperwork. They will go by whatever thats' on your marriage certificate, even if you didn't want to take his last name that was the perfect time to adjust it.

Gone but not Forgotten!
