DS-3025 from Knightsbridge

Hello Everyone,

Thankfully we have completed the K1 process with visa being issues on Nov. 7th. So now I am trying to education myself on the adjustment of status (AoS) process which is coming up soon. For those who have not read up on this, I would recommend you do as it is a little more complex than the K1 process and more expensive.

Question for those who had their medical at Knightsbridge. Were you given a form called DS-3025 (it's related to your vaccination record) at the end of your medical? My fiance doesn't recall receiving the DS-3025 and we need it for his AoS. I am wondering if he places a call to the Knightsbridge doctor if they might be able to provide one.

Does anyone have experience or have some guidance?

From others being in the same situation I do not believe Knightsbridge keeps any copies of your paperwork. If your husband doesn't have his DS-3025 then he will need to have a complete I-693 medical examination here in the US.

Knightsbridge keeps zero records on their immigration physicals. I asked them myself. They always give out the form DS-3025, so if I was placing a bet, it would be that your fiancé also got it.

He doesn't have to mail a photocopy with AOS. Here's a cut/paste I have posted many times.

What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly.
