hi! I am applying for AOS and have some problem in filling-out form I-864 (Rev. 01/15/06). Is there anyone who can provide me a sample form of I-864 affidavit of support (under section 213A of the Act)? I've searched VJ sample forms but they have different one.

Jan. 30, 2009 - Mailed I-751 to CSC

Feb. 02, 2009 - CSC Received Mailed I-751

Feb. 09, 2009 - NOA1 (I-751 Receipt Notice)

Feb. 13, 2009 - NOA2 (Biometrics Schedule)

Mar. 05, 2009 - Biometrics Done / Touched

Apr. 11, 2009 - Received Congratulatory/Approval Letter (04/07/09)

May 14, 2009 - Touched (Card Production Ordered)

May 20, 2009 - Touched (Approval Notice Sent --- WEIRD)

May 26, 2009 - Green Card Received.

Use the example form here as a template to understand how to fill in the newer one on USCIS site.

Also if your income exceeds the poverty line by 125% consider using the I-864EZ, it has less financial evidence requirements.
