I don't mean to hijack this thread but I have a question in regards to mailing the documents to NVC. I've completed my DS-260 form online and now it says I have to submit the necessary documents. I can't find the document cover sheet for it. I only have the AOS document cover sheet available to me. The only bar coded sheet that I can see is the confirmation page of the IV which has my information and NVC case number on it. Do I print this and include this in my documents when i mail it to them? It doesn't say anything on it about being a document cover sheet like the AOS. <<posted by canuks05

Dens Reply>>Just go back on that link when we're you able to make a payment for AOS and IV fee

when you click each one it will show on the bottom for an option

Could someone post a sample cover sheet (with confidential information redacted)?
