I found this on the CFO website: http://www.cfo.gov.ph/index.php?option=com_content&view=article&id=1347:for-filipinos-leaving-the-

For youth immigrants aged 13 to 19, the following additional documents are also required:

  1. Photocopy of parent's passport or valid identification card, if accompanied by parent/s during registration
  2. Letter of authorization from parent/s authorizing the guardian to register the child with CFO on their behalf (if accompanied by guardian during registration)
  3. Original and photocopy of DSWD travel clearance (click here for the new guidelines on issuance of travel clearance)

You can also find the requirements you asked about on this page. :) GL!

Yes your postal ID and passport will work for the two forms of ID. They both need to be picture IDs.

You will need parental consent letter, notarized, along with copies of your parents ID (postal ID will work). If your mother wants to accompany you that is fine also.

Hank's posted the requirements.

Aside from the requirements, you need your CONFIDENCE in your relationship. Never put your AGE in your head. Be prepared. I bring all the documents I brought at the interview, so that if they ask some documents I have all of them ready.

Read good reviews here in VJ forum, not so good reviews might help just take it as a warning but don't base your confidence on them.

The Guidance Counselor might grill you on your AGE but if you are confident you will do just fine.

Cheers.......
