missing papers for my medical exam,..

hi guys,.....

Do we need the NVC letter and the USEM letter for the medical exam at St. LukeS? please help.. i havent received anything from both the nvc and the embassy yet, even emails and my medical exam is on september 4... are these 2 forms badly needed?

If you have your us embassy interview confirmation that would serve enough so you can have your medical.....( In addition to ur online registration at SLMEC and other requirements) That's all i have when i've done my medical.... Although i brought NOA2 as well in case...

Not needed at all. Once you have the MNL # from the NVC, you can move forward with exam and interview. Make sure you use this time to get all documents in order while waiting!

Thank you! so this is the go signal then: this are the forms i got prepared for my medical:

1. 2 copies of online registration

2. 2 copies of appointemnt confirmation letter

3. NOA 2

4. Visa photos 3 pcs 2x2 size

5. medical records

6. previous chest xray film

Is this enough and good to go?

Do not forget the Health Certificate with previous vaccinations. Philippine City Health is a good place to get updated certificate that is accepted by ST Lukes. Also, try to get all the vaccines before you go to St Lukes to really save time and not have to return to St Lukes since they will only give a few vaccines at one time. Look at the US Embassy site for the Long list of vaccinations, Ouch.

Where do i get that health certificate? How important is this as part of the requirements?

the health certificate is the official medical document listing all your vaccines that you already had say as a child or recently. City Health in your area should give you that. You need an official certificate showing what vaccines you have had not just a written list. I highly recommend getting all the vaccines before you go to St Lukes you have enough time before your appointment.

Also, I had my fiancée go to a private doctor for some vaccines and then also the local Medical center which are less money. but it is City Health Office that gives the health certificates listing all the vaccines and medical history. At least for Zamboanga City that is how it worked for us.

Lately, SLEC is asking for the photocopy of the NVC letter or the US embassy letter of instructions ...so it would be best to have either of the letters on hand ...they asked the NVC letter when I had my SLEC medical last month.

...my fiancé receivd our NVC letter in USA about 10 to 15 days after we got our NVC number by phone, so hopefully ur fiancé will receive by mail the NVC letter before your Sept. 4 medical, just tell him to scan and email to you ASAP once he will receive it.

Regarding the vaccine record, it' is no problem if you don't have the record but if u have the vaccines record you will have minimal vaccines ...you cannot have vaccines in advance with a private dr now cause there should be interval of 30 days from one vaccine like mmr to 2nd shot of mmr ....I guess you will have 4 to 5 vaccines during your SLEC medical ...and girl, it would be very painful.

My fiancée didn't bring a health certificate and they never asked her for one in the vaccine interview. If you can't track one down it's not a critical item so don't sweat it. They will just give her the required vaccines. Also, they only asked for a copy of the appointment letter for the embassy but if you have the NVC letter it's okay. Better to be safe than sorry.

Not needed at all, not asked for SLEC. SLEC will give the necessary shots upon completion of the medical exam.

You don't need that at all. Just do you medical and have a nice day, SLEC will administer any shots that are required then issue you a 3025 listing the vaccinations.

(I see you have your appointment confirmation letter - that is what SLEC will ask for... so you are good to go)
