When my husband went to get his state ID, they pulled a report from DHS. It had his first and middle name on one line, and the middle name field was blank. So the lady said she had to hyphenate them since they were now both his first name. So, for example, he is Evan-Charles Jones and they printed it that way on his ID. I don't even know where to fix it! Has anyone heard of this before?

So, his K-1 visa and the I-94 have his name correctly? Is this something that just happened after you filed for AOS? Perhaps someone entered something wrong recently then. Make an InfoPass appointment to try and get his name corrected now so he can get his ID and does not end up with an EAD/AP and green card with the wrong name on them.
