Timeline

We need a passport for my fiancee's illegitimate child . I read the DFA website which was a bit confusing . What exactly would she need to obtain a passport for the baby ? The main document would be the birth certificate . Then she would need all her information , passport and ID's ? Consent form for the baby to travel aboard from the mother ? Anything else that she will need at the DFA ?

Are you trying to obtain a US passport for the child or a passport issued from the country the child was born in?

DFA is where one would go to get a Filipino passport. That is why this is in the Philippines forum.

The mother needs to present:

- the child's original NSO birth certificate (DFA will not accept the LCR certified true copy)

For additional ID, mother needs to present 3 valid ID's of her own like her NSO BC, passport and driver's license.

There's a list of acceptable IDs at the DFA website.

Consent is not needed as the mother has complete custody over her illegitimate child.
