Basic questions about I-864, G-325A, I-693, I-485

Hi,

I know this forum has lots of information, and I read most of it, but I still have a few(basic) questions:

Form I-864 It's my husband (U.S. citizen) who fills it out? It is a box 1.a. that he should check? He is also filling out I-130. His annual income is more than 125% of poverty line, which means that I do not have to include my U.S. income (have a small student job on campus), right?

Form G-325A for me (non U.S.) what should I mark in the section: "This form is submitted in connection with an application for:....." Should it be Other? AOS?

Form I-693 Part 2. Medical Examination Should I see a doctor prior to sending my AOS documents? Or afterwards I will receive a request to do biometrics <?> that many of you mention?

Form I-485 Which option should I check under Part.2 Application Type?? I know it may be obvious for you, but really, I have no idea - I am in F-1 now.

1. Yes your husband in the only sponsor. You don't have to add your income is he qualifies.

2. There is a box somewhere that says something like "becoming a Permanent resident."

3. Yes you see a USCIS-designated civil surgeon before and get the results of your exam on the I-693 and out into a sealed envelope. You send the sealed envelope to them in your packet. Biometrics is NOT a medical exam, just fingerprints and a photograph - different thing.

4. Option A.

8/17/10: INTERVIEW DAY (day 123) APPROVED!!

ROC:
5/23/12: Sent out package
2/06/13: APPROVED!

Just to clarify on this:

3. Yes you see a USCIS-designated civil surgeon before and get the results of your exam on the I-693 and out into a sealed envelope. You send the sealed envelope to them in your packet.

So now I should look for this civil surgeon and make this appointment here: https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV? Or should I send I-693 blank just with my personal info, and wait for an invitation to a medical appointment from USCIS? Sorry, I know it may sound basic for you, but for me it is like a black magic :)

It's all done by you. You find a Civil Surgeon at uscis.gov. You make an appointment for an "immigration medical." The CS fills out the form with the results and gives you a sealed envelope (do not open envelope). You send sealed envelope in with your initial package.

Call different CSs and get quotes for the medical. Try to find one around $200 or less.
