For section c, it asks the address of the benificiary twice. Which address should I put, her permanent one (Household book) or her temporary address that she resides most of the year?

Tough call...but my sense is you want to use the temporary address so it aligns with where she goes to school or works. This will require a police check covering both provinces so need to be careful she doesn't run into problems on the VN side. PM me if you want more info.

Ah, it's the same province just the temporary is in the city and the permanent is rural. She registered her temporary address as well.
