I have maiden and former married name....do I have to list them fully? first middle last for both? I hate making an attachment sheet for just this.

When Mary Jane SMITH married JONES then WILLIAMS, she would enter Mary Jane SMITH, JONES, WILLIAMS. Use full names, not initials on these forms, unless John Michael SMITH wants to indicate he goes by J. Michael SMITH etc. In that case, he would indicate "goes by J. Michael SMITH". Always type family names and countries in all CAPS and nothing else in ALL CAPS.

thank you so much for this. I was given the advice to put everything in caps on the form except first names thank you!

There is no purpose in using CAPS for given names, addresses, company names etc. but there is for family names and countries. You were given poor advice.
