I-134 affidavit of support questions

I have two questions about the affidavit of support I-134

1. For Fiancée visa, what do Put in line 11. Do I check intent, if so what do I put in? On the example they show on this site, it says no check mark and then "N/A (K1 VISA PROCESS FOR PERMANENT RESIDENCE)" Is this right?

2. At the bottom, the "oath or affirmation of Sponsor. there are two blanks bottom one is signature, but the one above where it says "I have read Sponsor and alien Liability...... there is a blank, do I put another signature there or just print my name? As I am now preparing this any help would be appreciated from anyone who has been approved and what they submitted on this form. It is going to Manila Embassy. Thank you

07/15/2013...... EAD card arrived in mail today smile.png

10/20/2013...... Green Card approved! NOA hardcopy received!

10/31/2013...... Green Card Delivered!!

07/21/15 90 day Window Opens

07/24/15 I-751 Mailed to Cali. Service Center
09/03/15 Biometeric scheduled and completed

01/26/16 ROC Letter arrived
01/30/16 10 yr Green Card arrived

1. I put intend, and wrote that I intend to cover all expenses for my Fiancé for the time necessary including, living expenses such as room & board, food, spending money etc. Our visa was approved so I don't think there is any issues if you check mark the box.

2. I left that side line blank. Since you sign the form below I didn't see the need to put anything on that line. Again the embassy accepted the form that way with no issues. If you feel uncomfortable you can always initial there.
