Timeline

Under section C (Information about your relative) question 14:

Do I need to put what kind of visa I used to enter the country?

Does the I-94 information have to be included ( I'm not sure if I still have a copy of it)

"Information about your relative" means they are asking about the person you are trying to bring over.

If that person is currently in the US, they want his/her I-94 info.

They are not asking about you. Since you are a USC, you wouldn't have an I-94 anymore.
