what is submitted with a AOS co sponsor affidavit?

i know their tax return is included, letter from there employer, bank statement, pay stubs, and another letter stating there support for my wife in our marriage but what else is needed? let me know if im missing something.. does the co sponsor need to give a copy of his passport or birth certificate as well? if so can it be either or?

They do not need to provide a letter stating they will support the foreign spouse. They need the I-864, most recent tax transcript(2012), letter from employer and/or most recent pay stub(s) plus proof of being a USC or LPR. A copy of their US birth certificate OR the bio page of their US passport will work.
