birth certificate ( petitioner )

birth certificate ( benificiary )

death certificate ( my dad 2nd husband )

marriage certificate ( my dad 2nd husband )

death certificate ( 1st husband )

marriage certificate ( 1st husband )

NBI

2 US passport ID

Biographic copy of passport

divorce decree ( petitioner)

And photocopies of all the originals as well..

My question is do we need to send out my mom's 1st husband death certificate and their marriage certificate? she's widowed. Both husbands passed away. Pls help I need to send my IV documents as soon as I clear this out. Thank you.

Yes, according to this:

6. MARRIAGE RECORDS (if applicable): For Philippine marriages, the copy of the marriage certificate must be printed on National Statistics Office (NSO) security paper. If the marriage ended by divorce, annulment, disappearance or death of the spouse, legal and/or civil documentation must be presented attesting to the termination of the marriage (annulment decree, death certificate issued by NSO and printed on security paper, a foreign divorce decree, or foreign death certificate).
