I am using the example third party affidavit found here http://www.visajourney.com/wiki/index.php/Affidavit as a starting point for an affidavit I will have someone sign.

My question is, for the statement

I am making this affirmation in connection with the application of [NAME] for a Visa for the United States of America. The contents of the affirmation are based on my own knowledge and are true.

Would [NAME] in this case be the beneficiary (foreign spouse) ?

Yes, the beneficiary is your spouse.

So in the statement mentioned above, [NAME] is the beneficiary's name, since he/she is the one who will receive the immigrant visa, in case of being approved at the end of the process.
