I have question related to the I-134 and a co-sponsor.

My mother has offered to co-sponsor my fiancee. My mom is retired and receives a pension and social security benefits.

For "I derive an annual income of ______ , should we add up the gross amount of both her social security (the type she paid into it while she worked) and her pension and put that in the blank? We have 3 years tax transcripts for her. Her income would be over 42K a year, so I know it should be more than the 125% for 2. My mom also owns the house that she and I live in, but we don't think that needs to be listed in the assets since her income is more than enough. I am currently laid off so my I-134 will be staying that and putting my current savings and checking account added up. I will also supply my fiancee with my W-2s from when I collected income and my letter stating I made no income in 2011, as well as bank statements of my savings/checking of the past 6 months. We are hoping my fiancees interview will be early December since her medical is in 1.5 weeks.

One other question... On the I-134 would relationship to sponsor be "future daughter-in-law" for my mother's I-134? Same for question 10 where it shows relationship?

"Sons Fiancee" will work.

Be sure to provide documents that prove how much she receives from each. No need to list any assets if she makes enough income. Leave that part of the I-134 blank. Also, make sure she provides proof of being a USC or LPR.

I have scanned and photocopied her birth certificate (both sides), though its a bit wrinkled since its an official old copy. The back has tape on it, though it is blank otherwise. lol. She has used it for other things and never had a problem with it so I'd assume it's fine. Its from 1958 so its the black looking one. It is legible in all the important places. The tax transcripts show her pension and social security on separate lines so that should be enough proof right? She doesn't get checks or anything everything is digital and direct deposited in her bank account.

I am guessing she gets the same amount each year, so showing her income on her tax transcript should be fine.

Ok, thanks. Also for question 11, should we put N/A (K1 visa process for permanent residence) and check neither?

Yes. Just like the example form here on VJ shows. You can alternatively check 'intend' and still write that on the line.

Would it be best to check intend? I was just not going to check anything and just fill it out as the example on here shows. She actually received more money last year and this year. We are giving 2009-2011 tax transcripts for her. I'm giving my tax transcripts from 2008-2010 and the letter of tax exemption for 2011 since I made no income.
