How should my fiance write her address for Question 16 on the I-129F?

For example, if her address is something like: 1-12-16 Tsukuda Nishiyodogawa-ku Osaka JAPAN 5550001

and the categories given are:

Number and street:

Town or City:

State or Province:

Country:

Which part would go where? Also, for this question she has to write in her native language, so is it better she write it in Kanji or Hiragana?

I'd appreciate any help.

I think this is what we did, would it matter if we put the Nishiyodogawa-ku part in the city box?
