Yes. The link is at the bottom of the page, second one of 3 links, DS-156: Nonimmigrant Visa Application (Use this form if you are applying for a K visa.)

Yes, they all need the forms, K-1 and K-2 applicants. Remember you need the DS-156 in duplicate for each and do not sign them until they tell you to sign them at the consulate.

Yes, all the forms filled out for the K-1/K-2 interview are filled out by and about the foreign fiance(e) and eligible K-2 children applying for the visa.
